A provider-patient relationship is only established after you have undergone a formal clinical evaluation and signed our specific Clinical Consent and Intake forms.
3. SERVICES AND BOOKINGS
We provide physical therapy services in Mill Valley, CA.
- Discovery Visits: Requesting a “Free Discovery Visit” is subject to availability and does not guarantee an appointment.
- Cancellations: If you book an appointment, you are subject to our clinic’s specific cancellation and “no-show” policy, which will be provided to you at the time of booking.
4. INTELLECTUAL PROPERTY
Unless otherwise stated, Marin Peak Physio and/or its licensors own the intellectual property rights for all material on the Site. All intellectual property rights are reserved. You may access this from the Site for your own personal use, but you must not:
