It’s a strange thing to get used to, and most people do get used to it.
You wake at three in the morning with a hand that feels dead, or full of pins and needles, or burning. You shake it out. After a minute or two it comes back. You go back to sleep, and by morning you’ve more or less forgotten about it.
Then it happens the next night. And most nights after that.
Night-time hand numbness is one of the most common nerve symptoms there is, and one of the most consistently ignored — partly because it resolves within minutes, and partly because it seems too minor to mention. But the nocturnal timing isn’t random. It’s a clue, and it usually points somewhere specific.
Why Night Specifically?
Three things converge while you sleep.
Position, held for hours. Awake, you shift constantly. Asleep, you can hold a wrist fully bent, or an elbow fully flexed, for an hour at a time — positions that narrow the spaces nerves pass through.
Fluid shifts. Lying flat redistributes fluid, and mild swelling in a confined tunnel raises the pressure on whatever passes through it.
Nothing to distract you. The same signal registers more loudly at 3 a.m. than at 3 p.m.
So the nocturnal pattern isn’t a separate condition. It’s the same compression problem, revealed at the time of day when the conditions are worst.
Which Fingers? That’s the Question
The single most useful thing you can notice is which fingers are affected. It narrows the field immediately.
Thumb, index, middle, and half the ring finger — the median nerve, most often compressed at the carpal tunnel in the wrist. Night symptoms are characteristic here. In fact, severe arm pain at night, later extending into the day, is a recognized part of the picture, and in more advanced cases there’s wasting at the base of the thumb with weakness of thumb movement.
Little finger and half the ring finger — the ulnar nerve, most often compressed at the elbow rather than the wrist. People with this frequently sleep with elbows bent, which is exactly the position that compresses the nerve.
Both hands, and also both feet, symmetrically — a different situation entirely, and one that points toward a systemic peripheral neuropathy rather than a local compression. This needs medical assessment and bloodwork.
A strip down the arm into specific fingers, with neck involvement — possibly coming from a nerve root in the neck.
Whole hand, or a pattern that doesn’t fit any of the above — worth an assessment rather than guessing.
What the Evidence Supports for Carpal Tunnel
Since this is the most common of the group, it’s worth knowing what actually works. Conservative treatment is regarded as first-line.
Night splints. Wrist splints holding the wrist in a neutral position are the most commonly preferred option and are recommended at all stages. The logic is straightforward — a neutral wrist gives the tunnel its maximum available space, and the splint prevents you bending into a compressive position while asleep. Night use is the priority; daytime use can help if symptoms occur then too.
Nerve and tendon gliding exercises. Randomized controlled studies have shown these contribute to symptom reduction and functional improvement. They’re specific movements designed to help the nerve and tendons move relative to surrounding tissue, and they’re dose-sensitive — done too aggressively they aggravate.
Activity and ergonomic modification, addressing whatever repetitive or sustained loading is contributing.
Corticosteroid injection, which can provide relief and is a physician decision.
Surgery. Surgical release is the definitive treatment for severe carpal tunnel syndrome with established medium- and long-term effectiveness. It isn’t without downside — pillar pain has been reported in up to 38% after decompression — but for severe or unresponsive cases it works well.
The honest caveat worth knowing: long-term results from conservative treatment remain uncertain in the literature, which is why persistent or worsening symptoms shouldn’t be managed indefinitely without review.
If It’s the Elbow Instead
For ulnar nerve symptoms — little and ring finger — the approaches differ.
Keep the elbow from bending fully at night. An anti-flexion splint, or a towel wrapped around the elbow, prevents the sustained flexion that compresses the nerve. This is often the single most effective change.
Avoid leaning on your elbows — desk, car door, armrest.
Nerve gliding, appropriately dosed.
Activity modification, particularly for anyone whose work or sport involves sustained elbow flexion or pressure.
Simple Things Worth Trying First
Change how you sleep. Try keeping wrists straight and elbows relatively extended. It’s harder than it sounds, which is why splints exist.
Note whether it’s one hand or both, and whether it’s always the same one. Consistent one-sidedness points toward a local compression.
Track how long it takes to resolve on waking. Seconds is different from twenty minutes, and the trend over weeks matters.
Look at what changed in the weeks before it started — a new job task, a new hobby, more time on a bike, a change in training, weight change, or pregnancy.
Why Not Just Live With It
Because nerves under sustained compression can change.
Prolonged compression can lead to reduced blood supply to the nerve and, if left long enough, lasting damage. The progression is usually slow and easy to normalize, which is exactly why people arrive years later with thumb muscle wasting and weakness that would have been avoidable.
The practical rule: intermittent numbness that resolves quickly is a warning worth acting on. Numbness that’s becoming constant, or any weakness, means it’s already progressed and shouldn’t be watched any longer.
When to Get It Assessed
Book an appointment for symptoms most nights for more than a few weeks; numbness that’s beginning to persist into the day; any weakness, clumsiness, or dropping things; visible wasting of muscle in the hand; symptoms in both hands; symptoms not improving with simple position changes; or numbness affecting your sleep enough to matter.
Seek prompt medical assessment for numbness with neck pain radiating down the arm; numbness alongside unexplained weight loss, fever, or feeling generally unwell; symmetrical numbness spreading in both hands and feet; or rapidly progressing symptoms.
Seek emergency care for sudden numbness on one side of the body, particularly with facial droop, arm weakness, or difficulty speaking. That’s a stroke pattern and time is critical — call 911 rather than waiting to see whether it passes.
Also worth mentioning to your physician: night-time hand numbness can be associated with pregnancy, thyroid disease, diabetes, and other medical conditions. If it appeared alongside other health changes, that’s relevant information.
A Note on Splints
If you’re going to try a night splint, get one that holds the wrist genuinely neutral rather than in extension — many off-the-shelf options have a metal stay that positions the wrist bent backward, which is the opposite of what you want. Worth checking before you spend eight weeks sleeping in something unhelpful.
